## Indications for Personal Rehabilitation

Personal rehab devices and services treat acute, chronic, and recurrent pain. Indications include but not limited to mild-to-severe pain.

### Where does it hurt?

Systemic painBack painHead and neck painLower extremity painUpper extremity pain

How Analgesic Healthcare describes it

**1\. Systemic Pain** Comprehensive relief for pain throughout the body.

A TENS unit works where the electrode pads are placed. It does not treat the whole body at once. For widespread pain from conditions such as arthritis or fibromyalgia, your clinician may pick one or two target areas. Medicare policy L33802 does not name systemic pain. Check your plan.

The Medicare TENS policy does not name this area; coverage depends on your plan and your clinician's documentation. As of the LCD revision effective January 1, 2024, it covers TENS for "chronic, intractable pain or acute post-operative pain" when criteria are met.

How Analgesic Healthcare describes it

**2\. Back Pain** Effective solutions for chronic or acute back pain.

A TENS unit sends mild pulses through pads near the painful area. For back pain, Medicare policy L33802 says TENS for chronic low back pain will be denied as not reasonable and necessary. Other plans differ. Your clinician decides if a trial fits you, and tells you where pads may go.

As of the LCD revision effective January 1, 2024, the Medicare TENS policy (L33802) says "TENS therapy for Chronic Low Back Pain (CLBP) will be denied as not reasonable and necessary." Other plans set their own rules; check with your plan.

How Analgesic Healthcare describes it

**3\. Head and Neck Pain** Targeted therapy to alleviate head and neck pain.

Head and neck pain needs extra care with a TENS unit. Medicare lists headache as not reasonable and necessary. The NHS says not to use TENS on your neck, mouth or eyes. University of Iowa Health Care says not to use it on any part of your head, face or throat.

As of the LCD revision effective January 1, 2024, the Medicare TENS policy (L33802) lists headache, and temporomandibular joint (TMJ) pain, as conditions for which TENS therapy "is not considered to be reasonable and necessary." Other plans set their own rules; check with your plan.

How Analgesic Healthcare describes it

**4\. Lower Extremity Pain** Treatments to ease pain in the legs, knees, and feet.

For leg, knee or foot pain, a TENS unit works where the pads sit. The NHS shows pads above and below a knee. The NHS and University of Iowa say not to use TENS on numb skin, which matters in foot neuropathy. Medicare policy L33802 does not name leg pain. Ask your clinician first.

**5\. Upper Extremity Pain** Specialized care for pain in the arms, shoulders, and hands.

For shoulder, arm or hand pain, a TENS unit works where the pads sit. Shoulder pads can sit near the chest, so pacemaker and chest limits matter. The NHS says not to use TENS with a pacemaker. Medicare policy L33802 does not name upper extremity pain. Ask your clinician first.

## The path to a TENS unit under Medicare

Five steps from the Medicare rules. Other plans set their own rules, so check with yours.

1. 1

    ### See your treating clinician

The clinician who treats the condition causing your pain is the one who orders the TENS unit.

[CMS LCD L33802, Transcutaneous Electrical Nerve Stimulators (TENS)](https://www.cms.gov/medicare-coverage-database/view/lcd.aspx?lcdid=33802)
2. 2

    ### A written order before delivery

Medicare requires a face-to-face visit and a written order that reaches the supplier before the unit is delivered.

    [CMS Policy Article A52520, TENS](https://www.cms.gov/medicare-coverage-database/view/article.aspx?articleid=52520)
3. 3

    ### The clinic sends the preauthorization

Your doctor or physical therapist sends Analgesic Healthcare the preauthorization form with your name, date of birth and insurance details.

    [The preauthorization form](https://analgesic.multisites.ai/preauthorization-form)
4. 4

    ### A trial period, for chronic pain

For chronic pain, Medicare pays for a trial of at least one month and no more than two, watched by your clinician.

[CMS LCD L33802, Transcutaneous Electrical Nerve Stimulators (TENS)](https://www.cms.gov/medicare-coverage-database/view/lcd.aspx?lcdid=33802)
5. 5

    ### Your clinician decides what comes next

If the unit helps, it can become part of your care. If it brings no relief, TENS is ruled out.
